Platelet Aggregation - Effect of Clopidogrel (Plavix)

Figure 1. 70 year old male with a history of coronary heart disease was scheduled for an elective inguinal hernia repair.His medications included daily aspirin and clopidogrel (Plavix) but he had stopped these 5 days previously as instructed. Platelet aggregation studies were requested to assess for residual drug-related platelet function inhibition. His platelet count was 208,000/ul. The image shown is the aggregation pattern and ADP release at a final concentration of 5uM ADP using platelet-rich plasma (PRP)from the patient and a normal control. Patient (Blue)shows an initial wave of aggregation followed by disaggregation beginning at 1 minute. His ADP release(Black) is 0 .The control PRP shows 75% aggregation (Red) and 1.3uM ADP release (Green).

Figure 2. The patient's aggregation pattern (blue) and ADP release (black) are similar to that seen with low dose ADP agonist. The control PRP aggregation and ADP are essentially unchanged from that seen with 5 uM ADP agonist.

Figure 3. The patient's PRP (blue) shows complete aggregation at 3 minutes with normal ADP release (black). These results suggest an absence of an aspirin effect on platelet function i.e. cyclooxygenase function is intact.

Figure 4. The patient's PRP (blue) shows a pattern and percent aggregation similar to the control PRP (red). ADP release is also similar in both samples (patient - black; control - green).

Figure 5. Complete platelet aggregation of both the patient's PRP (blue) and control PRP (red)are noted. Since this agonist causes platelet aggegation and not "activation" with relase of platelet contents, ADP release is not measured.
