Figure 1. 74 female was seen because of shortness of breath. She had undergone a mastectomy several months earlier for adenocarcinoma of the breast. A pleural effusion was noted and she underwent a thoracentesis. The cell count of the fluid included 43 RBCs/uL and 404 nucleated cells/uL. A low power view of the cytospin preparation is shown. Multiple groups of cells are noted and a moderate amount of cytoplasm in individual cells is evident.

Figure 2. This is a view of a representative group cells noted throughout the specimen. The vast majority of cells have the characteristics of reactive mesothelial cells with basophilic cytoplasm and benign-appearing nuclei. Distinct nucleoli can be seen in many of these cells. In addition, there are scattered lymphocytes and mast cells present.

Figure 3. The mast cells can be seen primarily at the fringes of the cell grouping. A single mast cell is also present in the left center in this group.

Figure 4. A higher power view of the previous image is shown. The basophilic granules characteristic of mast cells can be best seen in the mast cell in the bottom left.

Figure 5. In this view the multiple mast cells surround the mesothelial cells. Note degranulation in several of the cells.

Figure 6. A higher power view of the previous image is shown. Note in many of the mast cells the basophilic granules overlay the nucleus and can obscure nuclear borders.
