Langerhans cell histiocytosis

John Lazarchick, M.D.

Figure 1. Lymph node biopsy from a 16-year-old female who presented with massive cervical lymphadenopathy. The histology of the node was consistent with a diagnosis of sinus histiocytosis with massive lymphadenopathy (SHML). However, one area of the biopsy showed an infiltrate distinct from SHML. A low-power view of the lesion is shown. Small, bland appearing lymphocytes are noted intermixed with abundant eosinophils.

Figure 2. The nuclei of the cells are variably shaped ranging from cleaved, grooved, folded, indented, and even lobated. Cytoplasm appears to be abundant and is eosinophilic. Clusters of eosinophils are noted surrounding the lymphoid infiltrate.

Figure 3. The variation in nuclear contours of these cells is more evident in this view of the lesion. Classic "kidney bean" nuclei of Langerhans cells with a central groove are present.
