There is a marked leukocytosis consisting primarily of neutrophils. In addition large cells with basophilic cytoplasm, round to irregular nuclei and prominent nucleoli are also present (arrow).
Figure 2. A higher magnification of the previous image is shown. The heterogeneity in size and nuclear contour of the atypical cellular infiltrate is more evident. On flow cytometric analysis these cells marked as CD19, CD20 positive with kappa light chain restriction consistent with lymphomatous involvement of the peritoneum.
Figure 3. The neoplastic features of these cells are most evident in this image. The cells have extensive agranular basophilic cytoplasm, variability in nuclear contours and one or more prominent nucleoli. A mitotic figure is also noted. A subsequent gastric biopsy confirmed the diagnosis of diffuse large B-cell lymphoma (DLBCL) in this patient.
