HIV-related marrow granuloma

View larger version (149K): [in this window] [in a new window]   Figure 1. Patient is a 38-year-old African-American female with HIV/AIDS who presented with fever. Bone marrow examination was performed to rule out Mycobacterium Avium. A large perivascular granulomatous lesion is noted with normal hematopoietic elements abutting the granuloma.

At higher magnification the lesion has swirling pattern, is of mixed cell type with small lymphocytes, histiocytes, and eosinophils predominant but no multinucleated giant cells are present. Flow cytometry was negative for a B-cell or T-cell lymphoproliferative disorder.

View larger version (144K): [in this window] [in a new window]   Figure 3. The cellular composition is most evident in this view with small lymphocytes with irregular nuclear borders, histiocytes, plasma cells, and eosinophils. Stains for acid-fast organisms and fungi were negative.
