John Lazarchick, M.D.

Figure 1. A low-power view of a lymph node biopsy from a 28-year-old female who presented with a history of intermittent febrile episodes and generalized lymphadenopathy is shown. Normal nodal architecture is effaced by an expansion of the interfollicular area. There is a semblance of a nodular pattern to this expansion without evidence of fibrosis. Large number of lacunar-like cells are noted.

Figure 2. A higher-power view of the previous images shows numerous lacunar-like cells (arrows) surrounded by small-to-intermediate lymphocytes and scattered eosinophils.

Figure 3. The lacunar-like Reed-Sternberg cells noted at low power show extensive cytoplasmic and nuclear variation with mononuclear, classic bi-nuclear, and polynuclear forms present. There is a background of benign-appearing lymphocytes, scattered eosinophils, and an expansion of the vascular network.

Figure 4. In addition to a classic binucleated Reed-Sternberg cell (thick arrow), both momnuclear R-S variants and L & H cells (thin arrow) can be seen in this view.

Figure 5. Numerous eosinophils are present in addition to the lymphocytes and scattered plasma cells in this view of the biopsy. The final diagnosis is mixed cellularity Hodgkin lymphoma.
